Title
Resolution relative to approving a contract with Aurora Health Care to furnish Health Risk Assessments for represented employees and their spouses for a one year period at a cost of $54 per person.

Body
WHEREAS, the Horton Group, the City's Benefit Consultant, has provided a proposal from Aurora Health Care to provide Health Risk Assessments (HRA) for represented employees; and

WHEREAS, in order to be eligible for the reduced monthly health insurance premium contribution levels for the March 1, 2020 to February 28, 2021 insurance plan year, represented employees and their spouses must participate in a City sponsored Health Risk Assessment; and

WHEREAS, the cost will be $54 per participant; and

WHEREAS, the Common Council deems it to be in the best interest of the City of West Allis that the proposal of Aurora Health Care be accepted.

NOW, THEREFORE, BE IT RESOLVED by the Mayor and Common Council of the City of West Allis that the proposal submitted by Aurora Health Care to furnish Health Risk Assessments for a fee of $54 per employee be and is hereby accepted. Funding for this agreement is budgeted and available in the Health Insurance Fund, and these services will be charged to Account No. 602-5601-517.30-04.

BE IT FURTHER RESOLVED by the City of West Allis that the City Administrator is hereby authorized to negotiate and execute the contract and negotiate the terms and conditions substantially in conformance with the purpose(s) of this resolution.

BE IT FURTHER RESOLVED that the City Attorney be and is hereby authorized to make any substantive changes, modifications, additions and deletions to and from the various consulting Contract documents, including but not limited to all attachments, exhibits, addendums and amendments, as may be necessary and proper to correct inconsistencies, eliminate ambiguity and otherwise clarify and supplement said provisions to preserve and maintain the general intent thereof, and to prepare and deliver such ot...
