Title
Resolution authorizing the City of West Allis to enter into a contract with Superior Vision effective March 1, 2019 to provide the voluntary benefit of vision insurance for permanent full and part-time non-represented employees participating in the Total Benefit Package.

Body
WHEREAS, the Common Council of the City of West Allis has approved a Total Benefit Package for employees, and one of the components is providing a voluntary vision insurance policy on behalf of permanent full and part-time employees who participate in the Total Benefit Package; and

WHEREAS, the staff has met with members of The Horton Group ("Horton") to review proposals obtained by Horton for Vision Insurance and determined that the policy with Superior Vision is in the best interest of employees; and

WHEREAS, the proposal from Superior Vision has a four year rate guarantee.

NOW, THEREFORE, BE IT RESOLVED by the Mayor and Common Council of the City of West Allis that the proposal submitted by Superior Vision to provide the voluntary benefit of vision insurance for permanent full and part-time non-represented employees, beginning March 1, 2019, with the premiums paid by the participating employees, be and is hereby accepted.

BE IT FURTHER RESOLVED by the Mayor and Common Council of the City of West Allis that:

1. The City Administrator is authorized and directed to execute all related agreements and contracts on behalf of the City.

2. The City Attorney be and is hereby authorized to make such substantive and non-substantive changes, modifications, additions and deletions to and from the various provisions of the contract, including any and all attachments, exhibits, addendums and amendments, as may be necessary and proper to correct inconsistencies, eliminate ambiguity and otherwise clarify and supplement said provisions to preserve and maintain the general intent thereof, and to prepare and deliver such other and further documents as may be reasonably necessary to c...
