Title
Resolution authorizing the City of West Allis to enter into a contract with Reliance Standard Life Insurance Company for the calendar year of 2019 to provide Long Term Disability Insurance for permanent full and part-time non-represented employees for an annual cost not to exceed $75,000.

Body
WHEREAS, the Common Council of the City of West Allis has approved a Total Benefit Package for employees and one of the components is providing a Long Term Disability Policy on behalf of permanent full and part-time employees; and

WHEREAS, staff has met with members of the Horton Group to review proposals obtained by Horton for Long Term Disability Coverage and determined that the policy with Reliance Standard Life Insurance Company is in the best interest for employees and the City.

NOW, THEREFORE, BE IT RESOLVED by the Mayor and Common Council of the City of West Allis that the proposal submitted by Reliance Standard Life Insurance Company to provide Long Term Disability Coverage for permanent full and part-time non-represented employees for an annual cost not to exceed $75,000, effective January 1, 2019 to December 31, 2019, with the premiums paid by the City, be and is hereby accepted.

BE IT FURTHER RESOLVED by the Mayor and the Common Council of the City of West Allis that:

1. The City Administrator is authorized and directed to execute all related agreements and contracts on behalf of the City.

2. The City Attorney be and is hereby authorized to make such substantive and non-substantive changes, modifications, additions and deletions to and from the various provisions of the contract, including any and all attachments, exhibits, addendums and amendments, as may be necessary and proper to correct inconsistencies, eliminate ambiguity and otherwise clarify and supplement said provisions to preserve and maintain the general intent thereof, and to prepare and deliver such other and further documents as may be reasonably necessary to complete the trans...
