Title
Resolution establishing health insurance funding levels, employee health care premium contributions, and Health Savings Contributions for the plan year March 1, 2019 through February 29, 2020.

Body
WHEREAS, the City of West Allis must establish premium levels for its self-funded health insurance plan on an annual basis; and

WHEREAS, in consultation with the City's insurance consultant, The Horton Group, City staff have analyzed various premium level options for the 2019-2020 plan year for health insurance; and

WHEREAS, staff recommends establishing premiums for health insurance as outlined in the attached Recommended Premium Rates.

NOW, THEREFORE, BE IT RESOLVED by the Common Council of the City of West Allis that approval is granted:

1. To impose self-funded health insurance premiums for the 2019-20 plan year with Anthem as recommended in the attached Recommended Premium Rates.

2. To impose a monthly employee health insurance premium contribution of 12% for those who participated in an HRA in 2017 or for those employees hired after the HRA's were conducted in 2017 (for those who were employed during that time period but did not participate may submit a city approved form documenting medical certification with their open enrollment submittal); or a monthly employee health insurance premium contribution of 20% for those who did not participate in the City-sponsored HRA in 2017.

3. To provide the Health Savings Account contributions of $500 for a single and $1000 for a family for eligible employees/retirees who are enrolled in the Anthem HDHP.

BE IT FURTHER RESOLVED by the Common Council of the City of West Allis that:

1. The proper City Officials are authorized and directed to execute all related agreements and contracts on behalf of the City.
