Title
Resolution establishing health and dental insurance funding levels for the plan year beginning March 1, 2015.
 
Body
WHEREAS, the City of West Allis must establish premium levels for its self-funded health and dental insurance plans on an annual basis; and
 
WHEREAS, the City of West Allis fully-insured dental provider, Care Plus, updates premium levels on an annual basis; and
 
WHEREAS, City staff have analyzed various premium level options for the 2015 plan year for both health and dental insurance; and
 
WHEREAS, staff has established recommended premiums for both health and dental insurance.
 
NOW, THEREFORE, BE IT RESOLVED by the Common Council of the City of West Allis:
 
1.      To approve self-funded health and dental insurance premiums for the 2015 plan year as recommended per Schedule A.
 
2.      To approve Care Plus dental insurance premiums for the 2015 plan year as communicated per Schedule A.
