Title

Resolution relative to approving the Renewal Agreement between the City of West Allis and Care Plus Dental Plans, Inc. for its Prepaid Dental Program.

Body

WHEREAS, the City of West Allis has had a Prepaid Dental Program for many years; and,

WHEREAS, Frank Haack has recommended the renewal of the Program; and,

WHEREAS, it is in the best interest of the City to approve this renewal program for an additional year.

NOW, THEREFORE, BE IT RESOLVED by the Common Council of the City of West Allis that the attached agreement and proposal between the City of West Allis and the Care Plus Dental Plans, Inc. for its Prepaid Dental Program be and is hereby approved for the time period specified of March 1, 2006 through February 28, 2007.

BE IT FURTHER RESOLVED by the Common Council of the City of West Allis that the proper City Officials are authorized and directed to execute said agreement on behalf of the City.
