Title

Resolution relative to selecting Workforce Health for furnishing Health Risk Assessments and access to online results and articles for a one-year period at a cost of $60 per employee.

Body

WHEREAS, the Health Department, assisted by Willis, has analyzed proposals from four different firms to perform Health Risk Assessments for the City of West Allis; and

WHEREAS, subsequent to the evaluation of the firms, the Health Commissioner is recommending that the contract be awarded to Workforce Health; and

WHEREAS, the sessions will be held at various City buildings or at Workforce Health during the months of October and November; and

WHEREAS, the Common Council deems it to be in the best interest of the City of West Allis that the proposal of Workforce Health be accepted.

NOW, THEREFORE, BE IT RESOLVED by the Mayor and Common Council of the City of West Allis that the proposal submitted by Workforce Health for furnishing Health Risk Assessments for 2015 for a fee of $60 per employee be and is hereby accepted.

BE IT FURTHER RESOLVED that the Health Department be and is hereby authorized to enter into a contract for the aforesaid services.
