Title

Resolution relative to accepting the sole source proposal of Froedtert Workforce Health for providing health risk appraisals for an estimated sum of $26,510.00.

Body

WHEREAS, The Purchasing/Central Services Division has reported that it has received a proposal for employee health risk appraisals; and,

WHEREAS, The Common Council deems it to be in the best interests of the City of West Allis that the proposal of Froedtert Workforce Health be accepted; and,

WHEREAS, the Health Department has determined that this contract is inappropriate for competitive selection because of the past relationship with this company and their ability to provide data for the Health Department to compare aggregate result data from previous years.  This would hinder their ability to efficiently assess and plan for the health needs of City employees.

NOW, THEREFORE, BE IT RESOLVED by the Mayor and Common Council of the City of West Allis that the proposal submitted by Froedtert Workforce Health to provide 482 health risk appraisals at $57.50 for a total estimated sum of $26,510.00 be and is hereby accepted.

BE IT FURTHER RESOLVED, that the Purchasing/Central Services Division be and is hereby authorized to enter into a contract for the aforesaid services.
